ORDER:

1. In the GO sixth read above, guidelines were issued for effective implementationof Employees Health Scheme (EHS). Further, it was suggested that the FinanceDepartment would issue necessary instructions relating to the operation of fundsrequired for the scheme and establish a system for prompt payment to the hospitalsproviding services. Accordingly, the following guidelines are issued for streamlinedmanagement for EHS financial operations.

a. The Drawing and Disbursing Officers (DDOs) of all departments are herebyordered to deduct the monthly contribution due to the Employees’ HealthScheme (EHS) from the salary of the employees and forward the schedule ofdeduction along with the Pay Bills to the concerned treasury every month, viz.,DTO / STO/ PAO / Assist PAO, etc.

b. The Drawing and Disbursing officers are required to furnish a certificate alongwith the Pay Bills that EHS contribution has been deducted from the salary of allemployees for whom salary is claimed.

c. The Treasury Officer in charge shall carefully scrutinise the pay bills and ensurethat necessary EHS deductions are made in accordance with the provisions of thisscheme.

1d. Similarly, the District Treasury Officers shall deduct the contribution from thePensions and prepare a schedule of deductions from the pensions disbursed forthe month and enclose a certificate that deductions have been made in respect ofall pensioners.

e. The Director of Treasuries and Accounts is hereby designated as the NodalOfficer for the overall management of all finances relating to EHS, that includeinter alia, monthly deductions of the employee and pensioner contribution,depositing the contributions in the PD account under Head of Account 8342 -00-

120-00-11-003-000, for which the Director of Treasuries and Accounts shall be theadministrator, accounting, release of funds to the EHS executing agency, theNTR Vaidya Seva Trust, reconciliation, reporting and monitoring.

f. The Pay and Accounts Officer and Director of Works Accounts are required to transfer the contribution of employees towards the EHS to the Director of Treasuries and Accounts every month.

g. The Finance Department will issue Comprehensive Budget Release Order(CBRO) in the month of April every year, authorising the release of Government’s contribution to the Employees Health Scheme under Head of Account 2210 - 01 M.H. 001- S.H. (10) Government Contribution for Employees.

h. The Director of Treasuries and Accounts will draw an amount equivalent to the employees and pensioners’ monthly contribution to the EHS from the account mentioned above at (g) and transfer the same to the EHS – PD Account. In case

of employees on deputation / working on Foreign Service like Andhra Pradesh Vaidya Vidhana Parishad (APVVP), the procedure followed for the Contributory Pension Scheme (CPS) shall be followed in case of Employees Health Scheme.


i. The Government hereby authorize the Director of Treasuries and Accounts to transfer Rupees Fifty Crores (Rs 50 crores) to the PD Account of the NTR Vaidya Sevaas one time advance to establish ‘EHS Revolving Fund’ to be operated exclusively for the purpose of EHS. This amount shall be a permanent advance to the NTR Vaidya Seva for the purpose of EHS and shall be replenished every month or earlier based on the submission of certified statement of expenditure(SOE) by the Chief Executive Officer,Dr. NTR Vaidya Seva Trust. At any given time, the Trust would have funds adequate to finance two months of EHS payments in its PD account.


j. The Chief Executive Officer of Dr. NTR Vaidya Seva Trust shall settle the claims of the Hospitals relating to EHS within fifteen days of the claim being preferred by the hospital and there shall be no lapse in this regard. Further, the Vaidya Seva Trust shall not mix EHS implementation with other programmes being implemented by the agency.

k. The Director of Treasuries and Accounts and the CEO of Dr NTR Vaidya Seva Trust shall reconcile accounts relating to EHS every month before the fifteenth day of the following month and furnish reports to the government.

l. The Principal Secretary of Medical and Health Department and the Finance Secretary (Expenditure) will jointly review the implementation of EHS every quarter and take immediate remedial action as required to ensure smooth andstreamlined implementation of the EHS.

2. The Steering Committee of the EHS that includes all stakeholders will review the implementation of the scheme periodically and make necessary recommendations for effective implementation of the scheme. This procedure for operation of EHS guidelines shall become operational with immediate effect.

ORDER:
In the G.O., first read above, detailed guidelines have been issued on “Out Patient treatment for chronic ailments”, under Employees Health Scheme. In paras 2 &3 of the G.O., it is mentioned that the chronic OP treatment will be provided in “Notified Government Hospitals”, and that Chronic OP Treatment comprises of
doctor’s consultation, pharmacy, Laboratory service, Radiology services and clinical laboratory service in “special clinics” which will be open for EHS patients from 2 PM to 4 PM on week days. It is also mentioned that ‘Yearly health check up’ will be provided in these notified hospitals for the employees who crossed 40 years of age on 1st of August of that year.
2. In his letter 2nd read above, the Chief Executive Officer, Aarogyasri Healthcare Trust has informed that all the existing teaching hospitals, which are under the administrative control of Director of Medical Education have facilities to provide OP services for all the chronic OP packages. He has also stated that in the
District Hospitals which are under the administrative control of APVVP., 10 district hospitals alone have facilities to provide OP services for all the Chronic OP packages and in the remaining three districts viz: Vizianagaram, Nellore and West Godavari, the District hospitals will have to have tie –up with private hospitals for certain Chronic OP packages. He has also furnished the list of “Notified Government hospitals” where OP treatment for chronic diseases will be provided, and the private hospitals with which District hospitals of Vizianagaram, Nellore and West Godavari will have a tie-up for providing the facilities. The list is Annexed to this Order.
3. After careful examination, Government hereby declare the Government Hospitals included in the Annexure, as “Notified Hospitals” for providing OP treatment for Chronic diseases / yearly health check up under Employees Health Scheme. The Chief Executive Officer, Aarogyasri Healthcare Trust shall display the list in EHS-web portal for the benefit of the employees, pensioners and their dependent family members.

GO 134 AP Employee Health Scheme Modifications Amendments to the scheme
Cashless treatment to the Employees, Pensioners and their dependents
GO 134 HEALTH, MEDICAL AND FAMILY WELFARE (I.1) DEPARTMENT Dt:29/10/2014
AP Govt has issued GO 134, regarding the amendments in the Employee Health Scheme to provide cashless treatment to the employees of AP State Govt and pensioners and their dependents. The state Govt employees can avail treatment from varies empanelled network hospitals for the list of therapies as notified in the GO. The complete GO 134 as follows 
GO 134 AP Employee Health Scheme Modifications Amendments to the scheme 
1. In the G.O. first read above, that has been amended in the G.Os., 4th and 5th read above, the then Government of pre organized Andhra Pradesh State launched “Employees Health Scheme” with effect from 05-12-2013, to provide cashless treatment to the employees, pensioners and their dependent family members in empanelled network hospitals for the listed therapies notified in G.O.Ms.No.176 HM&FW (M2) Dept dated 01-11-2013, duly replacing the reimbursement of expenditure incurred for inpatient care subject to the provisions of APIMA Rules, 1972.
2. Consequent on reorganization of Andhra Pradesh, Government has reviewed the implementation of the Employees Health Scheme. After careful consideration of the matter, Government hereby issue the following orders in modification of the orders issued in the Government Orders first to fourth read above, with reference to the “Employees Health Scheme”
A. The Scheme, which was originally launched with effect from 05-12-2013, shall be implemented with all the following modifications, in a full-fledged manner, with immediate effect. Health cards to all the remaining beneficiaries shall be issued immediately. 
B. In the first phase the following categories of beneficiaries will be covered.
a) Serving employees:
(i) All regular State Government Employees, as defined under the Fundamental Rules (FR), including Provincialised work charged employees; and 
(ii) Provincialised employees of Local Bodies 
b) Retired employees:
(i) All Service Pensioners;
(ii) Family pensioners and; 
(iii) Re-employed service pensioners
C. The Employees / Pensioners health cards shall be seeded with Aadhar data, which shall be the basis for Biometric identification under Employees Health Scheme .
D. Steering Committee under the chairmanship of Chief Secretary, as envisaged in the G.O., first read above, shall be constituted immediately by including ten (10) members of the recognized associations of the NGO’s, Pensioner’s Gazetted Officers Association and proportionate member of officers to be nominated by the Government. The General Administration (Services) will issue specific orders defining the composition Steering Committee.
E. The Steering Committee shall review the implementation of the scheme periodically and make appropriate recommendations for improving the effectiveness and efficiency of the scheme to the Government.
F. Out of one thousand eight hundred and eighty five (1885) procedures notified in G.O.Ms.No.176 HM&FW (M2) Dept dated 01-11-2013, three hundred and forty seven (347) procedures, which were earlier reserved exclusively for Government network hospitals, shall be permitted for treatment in all Network hospitals i.e., both Government and private hospitals. This, will, however, be reviewed after 6 months of implementation for taking a suitable decision on reservation of these procedures for Government hospitals.
G. The following rates are accepted for the Percutaneous Transluminal Coronary Angioplasty (PTCA) procedure with Stent: (i) PTCA with bare metal Stent – Rs.55,000/-
(ii) PTCA with drug eluting Stent – Rs.65,000/-
H. The empanelled hospitals which have National Accreditation Board for Hospitals and Healthcare Providers (NABH) accreditation will be paid 25% over and above the package prices fixed for non-NABH Hospital, for surgical and medical procedures, which shall include pre-admission OP evaluation.
I. The deductions from the salary / pension of the employees / pensioners towards contribution under the Scheme, shall be effected from the salary / pension pertaining to November, 2014 payable on 01-12-2014. Medical reimbursement under APIMA Rules, 1972 will not be allowed for the treatment undergone on and after 01-12-2014. Finance Department will issue instructions to all the Drawing & Disbursing Officers, Pension Payment Officers / Sub-Treasury Officers accordingly.
J. The benefits enumerated above shall be implemented within the estimated annual budget of Rs.220 crores, which is derived from 40% contributions from employees / pensioners and 60% contribution by the Government.
K. The Finance Department will issue separate orders out lining the procedure and for advancing funds to the Aarogyasri Health Care Trust from the monthly contributions based on the utilisation and settlement of claims.
L. The Finance department will issue orders for reimbursement of expenditure incurred by Aarogyasri Trust towards claims payment and administrative expenditure under Employees Health Scheme between 1-11-2013 and 31-10-2014.
M. The expenditure incurred shall be reviewed by the Steering Committee headed by Chief Secretary after six months of implementation, and the Committee shall make recommendation on revision of contributions or any other changes if found necessary.
3. The General Administration Department, the Finance Department and the Chief Executive Officer, Aarogyasri Health Care Trust shall take necessary further action in the matter.
5. This Order issues with the concurrence of Finance Department vide their U.O.No.304/A1/EBS.V/2014, Dated .29-10-2014.

ANDHRA PRADESH GOVERNMENT IS INTRODUCING HEALTH CARDS FOR EMPLOYEES AND TEACHERS FOR CASHLESS TREATMENT.

Documents Required for Employee Enrollment:

  1. Adharcard /receipt: Obtain Adhar Card Number for the enrollment in in the ECH e-from. Scanned copy of Adhar Card or Andhar Receipt. If you don’t have Adharcard , go and get from the nearest Adhar Card Center.
  2. Passport Size Photo: A scanned copy of photo with dimensions 45mm x 35mm ICAO compliant should not exceed the size 200kb.
  3. Service Register: Scanned copies of first two pages , which is having name etc with clear visible.
  4. Disabled Certificate: if you are disabled, Scanned copy of your disability certificate.

Documents Required for Dependent Family members:

  1. Scanned Photo the dimensions 45mm x 35 mm ICAO complaint size color photograph of 200 Kb size.
  2. Scanned copy of Adharcard with clearly visible name and number. If not having Adhar card , produce scanned copy of adharcard receipt.
  3. Scanned copy of Date of birth certificate if the dependent family member is less than 5 years of age.
  4. If you disabled, Scanned copy of your disability certificate.

How to submit application form

  1. Open web portal www.ehf.gov.in
  2. Use Login at top of the right side of the page.
  3. Use your Treasury id as Username and get password from DDO or use forget password.
  4. method to get password or dial 104 for password assistance. Use login as employee in the dropdown provided
  5. Change your password after first login and keep the password confidential.
  6. After changing the password, read the instructions carefully in order to avoid mistakes.
  7. Click on the ‘Registrations’ then click on ‘Initiate Health Card’ after reading instructions.
  8. Click on the Aadhaar No and enter Aadhaar number/Aadhaar Enrollment Number
  9. Check the "Dated" column on the right hand side of the acknowledgment copy along with the time-stamp in the format: Date (2 digits)/month (2 digits)/year (4 digits), and hours (2 digits): minutes (2 digits): seconds (2 digits), a total of 14 digits. Eg: Date: 10/08/2013 16:48:44 should be converted as 20130810164844.
  10. Provide the number so obtained, in continuation to the 14 digit Enrollment number. The 28 digit Enrolment ID is made up of 14 digit Enrolment number and date & Time Stamp. Eg.1111152100210620130810164844.
  11. Enter , the 28 digit number in the ‘Adhaar Enrollment Number’ box and then click on "Retrieve Details", then the Application form appears after this enter the details as required .
  12. Open the enrolment form and fill up as per instructions there. Select your head of the department , Drawing and Disbursing Office unit, Category of your post from the dropdown lists in the application , submit the data, and attach the needed documents. The uploading documents size should not exceed 200KB. If it is above 200KB then edit it using Ms-Paint or other photo editable software like Picasa .
  13. Add family members by clicking ‘Add beneficiary’ and then upload photos.
  14. Before going to save the enrollment form verify the correctness of the details, because you can edit the details provided after submit.
  15. After successfully submitting the form you will receive confirmation message to your cell phone.
  16. Print the application from and sign and make scan copy and upload after re-login , complete the process by clicking the “Submit”.
  17. If your DDO rejects the application according to the information provided by you , then resubmit the information as stated above with full correctness.

How to receive Health Card:

After submitting successfully the employee information and take an approval from the DDO. You will receive the approval message to your cell phone as “"Health Card Enrollment Application Approved by DDO 11010308XXX (10digits). Then approach nearest CIC (Card Issue Center) like MEESEVA centers with all your family members for taking biometric finger prints.

Signed application form with declaration*After you enter all the details in the e-form, you need to print it and sign the form. Scan the form and prepare a soft copy. The declaration says that , you declare that (1) you agree to the entire Aadhar data of your family to be transferred to AHCT, (2) you are liable for disciplinary action for declaring ineligible family members as dependents, and (3) the information submitted is as per your service register and true to the best of your knowledge.
